What you will accomplish:
- Coordinate, schedule, confirm and manage virtual patient appointments
- Handle cancellations and resolve scheduling conflicts as they occur
- Train patients in using and troubleshooting tech issues pertaining to video calls
- Ensure smooth patient flow by providing education during scheduling/confirming
- Communicate with the patient and their care team through various channels (calls, text, chat, etc) to ensure optimal clinical outcomes
- Maintain a positive, empathetic and professional attitude at all times
- Document all interactions with the patient and family within our proprietary system and our EHR
- Manage tasks for patients as needed (i.e. schedule appointments, collect consent, send documents, etc)
- Perform other duties as required or assigned
- Data entry within operating dashboards, reporting and workflow platforms
- Ensure Data Quality and Accuracy
We are seeking a compassionate and driven individual who has:
- Fluency English and Russian (required)
- 2+ years of experience working in a healthcare setting (required)
- 2+ years experience helping to coordinate care and services for members of a fragile population (required)
- Experience communicating with patients via phone/telehealth (required)
- Knowledge and understanding of chronic care management processes (required)
- A customer service mindset for both internal and external customers (required)
- Experience with documenting in electronic medical records (required)
- A love for the elderly and ensuring they remain healthy at home
- Ability to multitask
- Ability to perform duties as assigned or requested
- Comfortable in a dynamic and always evolving startup environment

The pay for this position will be $20 per hour based on experience.
The referenced salary range is based on the Company's good faith belief at the time of posting. Actual compensation may vary based on factors such as geographic location, work experience, market conditions, education/training and skill level.
